This 70-year-old male presents with circulating blasts. A peripheral whole blood sample is submitted for flow cytometric immunophenotyping using ClearLLab 10C Panels.

Compared with normal B cells, the expression of CD5, decreased CD20, and lambda light chain restriction of low intensity are aberrant.

The immunophenotype of the abnormal population is in keeping with abnormal immature T cells, i.e. T-lymphoblasts.

Compared with normal mature T cells, the decreased expression of CD2, CD5 and CD7 with increased CD16 and CD56 is aberrant.

Compared with normal CD34-positive progenitors, the near-uniform expression of CD7 and decreased expression of CD13 is aberrant.

Compared with normal mature monocytes, the decreased expression of CD13 and CD14 is aberrant.

Compared with normal B cells, the expression of CD5, decreased CD20, and lambda light chain restriction of low intensity are aberrant.

Compared with normal B cell precursors, the expression of increased CD10, dim CD13, dim CD33, CD34 on a subset, and CD123 are aberrant.

Compared with normal peripheral blood, the presence of an immature T cell population in the peripheral blood is aberrant, with co-expression of CD4 and CD8, mildly decreased CD5, and dim CD45.

The blasts are in the CD45dim gate and express CD33, CD34, CD117 and HLA-DR. The blasts also abnormally express CD10 and CD56 (in a subset), while negative for CD19.
